Federal Poverty Level (FPL) Guidelines
HealthWell disease funds have a household income limit of either 300%, 400% or 500% of the Federal Poverty Level (FPL). To verify the FPL of a disease fund, please check the Household Income Limit on the specific disease fund page.
To determine if you are eligible for assistance based on your household income, please use the FPL Calculator.
Please Note: If you live in a city or state where the cost of living is high, we will consider a cost-of-living index of 120 or above for an adjustment.
For patients in Puerto Rico or other US territories, we will only consider the household income limit at 400% and 500% of the FPL, depending on the Fund. Contact us at (800) 675-8416, M-F, 9 am to 5 pm EST., for more information.
